Eldercare Partners is a non-proft organization providing information, referral and professional services to help seniors and their family caregivers cope with the challenges of aging.
Our services include:
Family Caregiver Services - Our Family Consultants offer information, support, community resources, coaching and counseling on how to manage the dynamic nature of the caregiving relationship. Family Caregiver Services are fee-based on a sliding scale.
Geriatric Care Coordination - Our care managers are professional social workers that provide a comprehensive assessment which evaluates physical, emotional and cognitive functioning, living arrangements and environmental matters. Using the assessment, the care manager will identify and coordinate supoprt services and monitor ongoing needs. Geriatric Care Coordination services are fee-based. Information and Referral Services - Our free website includes a comprehensive listing of special events, workshops and support groups for family caregivers throughout the 7 county metro area.
